q     Polyethylene sheets(for numerous uses)

q     COPIES OF MEDICAL RECORD AND EVALUTION FORMS FOR EACH SCOUT AND LEADER (THE FORMS MUST BE COMPLETED AND SIGNED BY A PHYSICIAN AND THE MEDICAL RECORD SIDE MUST BE COMPLETED AND SIGNED BY PARENT OR GUARDIAN).

q     A COPY OF ALL PERSONAL/FAMILY INSURANCE CARSDS ATTACHED TO THE PHYSICAL FORM.

 

All equipment should be packed and clearly marked with the owner’s name and troop number.
